    Question How to get old LIC Agency details?

    In 2001, I was working as an LIC agent but due to not missing the target, my agency was terminated by LIC. Now again I wanted to start agency but with Health Insurer - Max Bupa. I have already under gone 100 Hours training passed it with flying colors. Now when I am filing the Agency - "FORM IA - APPLICATION FOR APPOINTMENT TO ACT AS AN INSURANCE AGENT", It is asking about details for my previous agency (please see below for details)

    Furnish the details of any insurance agency in force or ever hold by the applicant:

    Name of the Insurer
    Agency code Number
    Date of Appointment as agent
    Date of cessation of Agency
    Reason for cessation of agency

    *Please attach Agency cessation letter issued by the insurer
    I don't remember any of the above. Is there any way of getting this from somewhere?

    Best way is to contact LIC or your LIC Development officer or check old policy records of your customers.

    I heard that there is a website on which you can check after put some of your details e.g. name, dob etc.

    There is a IRDA website - http://www.irdaonline.org/PublicAccess/LookUpPAN.aspx
