In 2001, I was working as an LIC agent but due to not missing the target, my agency was terminated by LIC. Now again I wanted to start agency but with Health Insurer - Max Bupa. I have already under gone 100 Hours training passed it with flying colors. Now when I am filing the Agency - "FORM IA - APPLICATION FOR APPOINTMENT TO ACT AS AN INSURANCE AGENT", It is asking about details for my previous agency (please see below for details)
I don't remember any of the above. Is there any way of getting this from somewhere?Furnish the details of any insurance agency in force or ever hold by the applicant:
Name of the Insurer
Agency code Number
Date of Appointment as agent
Date of cessation of Agency
Reason for cessation of agency
*Please attach Agency cessation letter issued by the insurer
