What is the procedure of tranfer of Post Office - MIS (PO MIS) from one Post office to another?

I have opened a MIS at Jaipur, I would like to transfer it to Mumbai now. Is it possible to transfer my Post office MIS from one post office to the other? If possible, Could you please let me know the procedure of the same.

You will have to submit SB-10 (B) form to transfer the account.

1- Write an Application to Transfer
2- Fill up the Account Transfer Form - SB-10 (B)
3- Original PPF passbook
4- Also submit and carry originals of following documents with you -
a) Photocopy of PAN
b) Address Proof
