I could not get your query. If you are asking how to raise claim for passport loss then lodge a FIR and call the customer care no of the insurance company provided on the policy. Then do what they tell you.

1- If you're abroad, report any passport losses or thefts to the local police in the country you're in as soon as possible. This will reduce the risk of anyone else using your passport or your identity.
2- You should also report the theft or loss to the Indian embassy, consulate or high commission of the country you are in.
3- Then inform the insurance company about the loss.
